Introducing The PRIME Method: a practical approach to physiotherapy

Create the window. Own the movement. Build the capacity.

When you come to physiotherapy, you want to understand what is happening, what you can do about it and how treatment connects to the things you want to get back to.

I developed The PRIME Method to make that connection clearer. It is my clinical reasoning framework at Obsidian Physiotherapy, not a fixed programme that every patient follows in the same way.

What does PRIME stand for?

Pinpoint. We explore what is preventing you from moving and functioning the way you want to. Your history, physical assessment, activity demands and relevant lifestyle factors guide the plan. Assessment continues throughout your care.

Regulate. We consider the conditions that support movement and recovery. Depending on your needs, this may include education, pacing, breathing or strategies around sleep and stress. Not everyone needs the same regulation work.

Improve. We address relevant physical limitations. Hands-on physiotherapy, targeted mobility or motor-control work may create an opportunity to move differently. Where useful, we test and retest to understand your response.

Mobilise. We put that opportunity into practice, building from simple movements towards meaningful tasks. The key loop is Improve → Mobilise → Retest. If a change does not carry over, we investigate why.

Enhance. We progressively build strength, endurance and capacity for everyday life, work or sport. The aim is an increasingly capable and confident person, rather than a perfect-looking movement.

Hands-on treatment is part of the picture

Within PRIME, manual therapy is a tool chosen through assessment. A useful improvement in symptoms or mobility gives us an opportunity to practise, move or load differently.

Manual therapy creates an opportunity. Movement teaches the body what to do with it.

Progress has milestones, not a deadline

There is no rule that you spend two weeks in each stage. We look for relevant changes: clearer understanding, better self-management, improved movement control, increased activity tolerance or growing strength and confidence.

Several parts of PRIME can happen together. You may be building strength while also working on recovery habits. If your response changes, we can revisit earlier work. Your plan should adapt to you.

A framework for whole-person physiotherapy

PRIME supports evidence-based physiotherapy. It considers physical structures, movement, pain, nervous system state and relevant psychological or lifestyle influences together. It does not assume every problem begins in the nervous system and is not a promise of a cure.

The deeper aim is to understand your body better and develop more confidence managing movement and responding to pain, with less dependence on your clinician over time.
